In consideration of being allowed to participate in anyway in the Bootcamp2getinshape/CampHustleHard/GetInShape2 LLC programs, the undersigned acknowledges, appreciates and agrees that:
1) The risk of injury from the activities involved in this program, while particular rules, equipment, and personal discipline may reduce risk, the risk of injury does exist; and
2) I knowingly and freely assume all such risks, both known and unknown, and assume full responsibility for my voluntary participation; and
3) I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any usual significant hazard during my presence or participation, I will remove myself from participating and bring such to the attention of the nearest staff member immediately; and
4) I hereby release and hold harmless, GetInShape2 LLC/Camp HustleHard/Bootcamp2getinshape (collectively the “Releasees”) with respect to any and all injuries, disabilities, loss of life or damage to person or property occurring or arising out of participation in the program.
I have read this release of liability and assumption of risk agreement, fully understand its terms, understand that I have given up substantial rights by signing it, and sign it freely and voluntarily without any inducement.
5) I consent to the use of my photograph or other likeness in the promotional and other materials of GetInShape2.com/CampHustleHard LLC without payment or other consideration made to me.
If participant is under the age of 18, this
certifies that I, as parent/guardian with legal responsibility for this participant, do consent and agree to his/her release as provided.
